Comfort, confidence and independence all depend on having the right mobility support in place. As health conditions change over time, so do mobility needs. Equipment and aids that were once suitable may no longer provide adequate support, making everyday activities more difficult and increasing the risk of falls.
Our physiotherapists carry out comprehensive assessments within the care home, evaluating your loved one in their own environment. This allows us to identify any barriers to safe movement, recommend the most appropriate aids and equipment, and ensure every individual has the support they need to move as safely and confidently as possible.

What to Expect from Your Assessment?

Each assessment consists of a thorough evaluation of the individual’s muscle strength, current mobility, use of aids, flexibility, taking into account their medical history and cognitive status to establish the needs and support required.

Mobility Assessment

A detailed assessment of how an individual walks, their stability, walking pattern, pace and overall safety when moving around. This helps identify any factors that may increase the risk of falls and guides recommendations to improve mobility and confidence.

Transfer Assessment

Assessment of how safely and independently an individual can move between a bed, chair and toilet, with or without mobility aids or specialist equipment.

Strength & Flexibility Assessment

Assessment of muscle strength and flexibility to identify limitations affecting mobility, stamina and exercise tolerance as well as overall wellbeing.

Balance & Stability

Reviewing balance and stability in sitting position, standing, walking and changes in position to understand how safely and confidently a person is able to move.

Equipment Review & Progression

Checking existing mobility aids, including progression from Sara Stedy and transfer equipment through to walking frames and walking sticks, to ensure they remain safe, appropriate and suited to current needs.

Personalised Equipment Recommendations

Following the assessment, our physiotherapists provide professional recommendations for the most appropriate equipment and aids. We work closely with families and care home staff to ensure the right equipment is in place and being used correctly.

Walking Aids

Walking frames, three and four wheeled walkers and walking sticks are assessed for correct height, type and suitability for the individual's abilities.

Transfer Equipment & Hoists

Assessment of transfer from bed to chair to wheelchair by use of aids such as slideboards, standing aids and hoists to ensure safe and dignified transfers for both individuals and care staff.

Bed & Chair Positioning Aids

Advice and recommendations for bed rails, grab rails, stair rails and pressure-relieving cushions to support those with limited mobility or at risk of pressure injuries.

Referral & Background Review

We review the individual's medical history, care plan and any existing equipment prior to the assessment visit.

On-Site Assessment

Our physiotherapist visits the care home and carries out a thorough assessment of mobility, strength, balance and equipment needs in the individual's own environment.

Personalised Plans

A tailored rehabilitation plan is devised based on the assessment, outlining the equipment and mobility recommendations.

Collaboration with the Care Home Team

We work closely with the care home team, advising on safe techniques to support the individual with any new equipment or mobility aids for the progression and maintenance of their abilities.

Family Engagement

We communicate with families on a regular basis, keeping them informed about their family member's progress and any concerns that may arise.

Most assessments take between 45 minutes and one hour, depending on the individual's needs and complexity.

Referrals can be made by family members as well as care home managers.

Will the physiotherapist liaise with the care home staff after the assessment?

Yes, following the assessment, our physiotherapist will work closely with the care home team to ensure any recommendations are understood and implemented safely.

How often should a mobility and equipment assessment be carried out?

We recommend a review whenever there is a change in a resident's condition, following a fall or hospital admission, or at least annually for those with progressive conditions.

Can family members be present during the assessment?

Absolutely, we encourage family members to be present and involved in the assessment process.

Is the assessment available for both permanent and respite residents?

Yes, our services are available for all individuals living in a care home, whether on a permanent or respite basis.
